The signs of varicocele on an ultrasound will include veins that are wider than 3 millimeters with blood flowing incorrectly during the Valsalva maneuver. The scrotal ultrasound will reveal the size of the testicles. It also guides your urologist on the way to treat you.

Your urologist may also order other scans to rule out any other cause of varicocele, such as the existence of a tumor that compresses the spermatic vein. After analyzing the cause of your varicocele, your urologist will plan further treatment.

  1. Surgery-
    During the surgical process, the doctor will focus on blocking the blood flow into the pampiniform plexus veins. The procedure can include- 
  1. i) Microscopic varicocele-

 In this technique, the surgeon will proceed with making one centimeter of incision above the scrotum. The procedure is done under a microscope and includes ligation of all small veins and spares the vas deferens, testicular arteries, and lymphatic drainage. The complete procedure can take 2 to 3 hours. The doctor will usually discharge the patient on the same day.

  1. ii) Laparoscopic varicocelectomy-

    In this technique, your surgeon will insert thin tubes in the abdomen and proceed with ligation of veins. Since the number of veins in the abdomen that shall be ligated is lesser than the ones in the scrotum, this technique will take less time. It will last for 30 to 40 minutes.   1. Percutaneous Embolization-

The interventional radiologist will perform embolization. In the process, the medical professional proceeds with an injection of contrast through a tube with x-ray guidance to reach the source of the problem. The procedure aims to find all veins leading to the varicocele. To block the blood flow to the varicocele and scar them down, the medical staff will use coils, plugs with or without sclerosant.

This procedure is done either with local anesthesia or under light sedation. It can last from 45 minutes to an hour.

  1. Before the procedure-

Your doctor will also introduce gas into your abdomen to allow more space for the procedure. Moreover, he will insert surgical tools through small cuts and use them to cut any in large veins that block blood flow. To finish the procedure, the doctor will seal off the ends of the veins by small clamps or by cauterizing them with heat. He will further remove the tools and laparoscope once the cut veins are completely sealed.

Surgery work will take around one to two hours and you will get the discharge probably on the same day itself.

  1. After the procedure:

There are certain instructions in medicines your doctor will give you. You will need to take any medication or antibiotic that your doctor prescribes. Your doctor will also advise you to take painkillers to manage your pain after the surgery.

Further, your doctor will instruct you to clean your incisions but you can get it done by visiting the hospital. You will have to apply an ice pack to your scrotum for 10 minutes several times a day to control the swelling.

The other precautions that are advised include:

  1. You cannot have sex for nearly two weeks after the surgery.
  2. Moreover, you are not allowed to perform any hard exercise or lift any heavyweight.
  3. The patient cannot swim or take a bath by immersing your scrotum in water.
  4. Your doctor will ask you to not drive or open any machinery.
  5. You should not strain yourself when on a poop. In case you have constipation, your doctor will prescribe your stool softener.
